The Retinal Eye Screening Referral Form is available as part of the Teleophthalmology Program to improve the access to, and rates of, retinal assessment and follow-up for patients with diabetes.
 
The Collaborative Eye Care Guidelines For The Management Of Persons With Diabetes is a collaboration between the Ontario Association of Optometrists and the Ontario Medical Association that defines the roles for Family Physicians, Optometrists, and Ophthalmologists in the caring for patients with diabetes.
