Self-management refers to the activities that an individual can do to live well with one or more chronic conditions. These include gaining confidence to deal with medical needs, everyday roles & responsibilities, and emotional issues.The Central West Self-Management Program offers the following free Self-Management Program opportunities:
 
For individuals living with, or at risk for, chronic conditions:

Living a Healthy Life Program-Includes three evidence-based workshops that have been developed and licensed by Stanford University.
 
These are as follows:
• Chronic Disease Self-Management Program
• Diabetes Self-Management Program  
• Chronic Pain Self-Management Program
• Online Chronic Conditions Self-Management Program 
 
 
These programs are offered in workshop format, and run for 2.5 hours, once a week, for 6 weeks. These programs help support and help build peoples’ confidence in their ability to manage their health and maintain active and fulfilling lives by teaching people how to set achievable goals, solve problems, and develop an understanding of their symptoms.

For Health Care Providers:
 
Brief Action Planning is a FREE, highly structured, client-centred self-management support technique. Composed of a series of 3 questions and 5 skills, BAP can be used to facilitate goal setting and action planning. Each skill of the BAP tool is based on literature and the evolving understanding of what works and does not work to help people change. BAP is supported by evidence from behavioural science and self-management support that is grounded in the principles and practice of motivational interviewing.
 
Choices and Changes is offered through the Institute for Healthcare Communication. This communication workshop assist health care providers to deliver patient-centered care by enhancing their skills in self-management support and influencing healthy behaviour change when working with patients.
 
These workshops are a great opportunity to enhance client-centered care.
