Online CDSMP
What is the Online Chronic Disease Self-Management Program?
Developed by Stanford University, the Chronic Disease Self-Management Program is now also offered online to individuals across Ontario. This program was designed to help people with chronic health conditions overcome daily challenges and to better manage their health. It is a highly interactive online workshop.
Who should attend the workshop?
Adults with any type of long-term health condition as well as their friends and loved ones are welcome to participate. The program is broadly accessible, allowing individuals with any level of computer skills to participate from any computer with an Internet connection, even a dial-up connection.
What does it cost to attend?
FREE to all!
How long is the workshop?
The workshop is 6 weeks. Participants log on at their own convenience 2-3 times per week for a total of about 2 hours per week.
Who leads the workshop?
The workshop is led by two trained online leaders who live with chronic condition themselves or care for someone that does.
Workshop Details:
- Groups of about 25 participants
- Easy to follow
- Use your own computer with internet connection
- New lessons added each week
- Confidential and anonymous, the program takes place on a secure website
- Participants set their own goals and make a step-by-step action plan to help them feel better and start doing the things they want to do again
