Become a Peer Leader in the Chronic Disease Self-Management Program
Are you living with a chronic health condition, care for someone who does, or passionate about helping others? Are you comfortable speaking in front of small groups and interested in helping others to build their skills to live well with a chronic health condition. If so, we have a Peer Leader Training opportunity for you!
What is the Chronic Disease Self-Management Program?
This evidenced based workshop originally developed by Stanford University runs for 2.5 hours, once a week, for six consecutive weeks and is led by two Peer Leaders. The workshop helps participants develop new tools and skills to manage the symptoms and feelings that can result from living with chronic health conditions. Workshop topics include: actions plans, problem solving, physical activity, nutrition, relaxation techniques, positive thinking and working with healthcare providers.
What is the Chronic Disease Self-Management Program PEER LEADER TRAINING?
This 4-day training is currently being offered at no cost and will qualify you to lead Chronic Disease Self-Management Programs in the community with Leader Certification. This is a great opportunity to gain valuable experience in the field, enhance your skills, and help others.
Who Should Attend Peer Leader Training?
We are seeking volunteers within the Central West Region (Brampton, Malton, Dufferin County, Caledon, Woodbridge, Rexdale) to become Peer Leaders through certification with the Chronic Disease Self-Management Program. We accept a wide range of applications—from individuals with healthcare education and experience to those individuals with little or no experience. We are seeking English, French, Punjabi, and Hindi speaking Peer Leaders.
What is the Commitment after Peer Leader Training?
As a trained Leader you commit to deliver 2 workshops each year. In appreciation for your time an honorarium in the amount of $180.00 is paid to each Leader upon completion of a full 6 week workshop series.
When & Where is Training?
Training takes place once per year. At this time, we have completed our training for Winter 2020. We will post details for the next training when they become available.
