We are currently seeking volunteers within the Central West Region to become Peer Leaders through certification with Stanford Chronic Disease Self Management Program. These best practice educational programs help participants take control of their chronic health conditions and live life to the fullest.
Applicants do not require healthcare education or experience! Ideally, our leaders are individuals who enjoy learning new concepts and teaching others to build their self-management skills. Leaders may be living with chronic conditions themselves.
This opportunity is currently being offered free of cost. Participation in the 4-day Leader Training Program will qualify you to lead Self-Management Programs in the community with Stanford Leader Certification. A detailed Leader’s Manual is provided to each graduate and our team will be assisting you every step of the way.
A Self-Management workshop series is facilitated by two Leaders and is designed for small groups. Programs are delivered through 2.5 hour workshop sessions that are scheduled once per week, over 6 consecutive weeks with a follow up mentorship session.
