Living a Healthy Life Workshops
 
Do you have diabetes, heart disease, kidney disease, pain or any other chronic condition? Are you at risk for diabetes? Do you need help making changes to improve your health?

Many of us live with one or more chronic conditions. Although these conditions are different, the challenges and concerns are often the same. Having a long-lasting health condition can impact a person’s physical, mental and emotional health. The good news is that people living with one or more chronic conditions can manage their health by learning positive self-management skills.

Living a Healthy Life are three workshops based on self-management principles and developed by Stanford University:
  • Chronic Disease Self-Management Program
  • Diabetes Self-Management Program
  • Chronic Pain Self-Management Program
These workshops:
  • Are offered FREE of cost
  • Run for 2.5 hours, once a week, for 6 consecutive weeks.
  • Led by community volunteers
  • Offered in various locations throughout the community
Workshop participants learn how to set achievable goals, solve problems, and develop an understanding of their symptoms during this group based program.
 
Program graduates commented that, “the workshop helped me to realize that I am not alone – that there are so many others in the same situation with similar issues,” and “by the end of the workshop I felt I had developed the coping skills I needed, and I found that very comforting.”

People living with chronic conditions and their partners or caregivers are welcome to attend.
