Common Referral Form For Diabetes Education
Education has been recognized as key to the management of diabetes. Health professionals working in diabetes education include registered nurses, registered dietitians, and social workers who specialize, not only in the disease state, but also in facilitating behaviour change and self-management. Diabetes Education Programs collaborate with healthcare providers, patients, and their families to assist in managing the patient’s condition
